This form (a Word document) allows you to book teachers in advance. You can download it, fill it in, save it and email it to us as an attachment. Helpful tip: use "save as" and replace the date with today's date in your filename. Or if you prefer you can simply print it out, fill it in and fax it to us. Don't forget to fill in the part with the name of your school and your contact details!
